Our nurses are committed to ensuring each patient’s physical comfort,
managing pain and symptoms, and coordinating care across all services and disciplines for both the patient and family.

Physician Services

Our Hospice Medical Director oversees the care team and collaborates closely with the patient’s primary care physician to ensure the highest standard of care. While our physicians do not conduct home visits, our experienced nurse practitioners are available to visit patients at home as needed. We provide on-call support 24 hours a day to address any medical concerns that may arise.

Hospice Aides

Responsible for providing assistance with the patient’s personal care.

Licensed Clinical Social Workers (LCSWs)

Our Licensed Clinical Social Workers provide compassionate emotional and psychosocial support to patients and their families. They assist with coping strategies, advance care planning, and navigating end-of-life decisions. LCSWs also help connect families to community resources, provide grief counseling, and serve as advocates throughout the hospice journey. Their goal is to ensure that patients and loved ones feel supported, understood, and empowered during this time.

Medications

Our hospice team expertly manages medications to ensure effective pain control and symptom relief. We are committed to maintaining each patient’s comfort around the clock, with continuous assessment and timely adjustments as needed. All medications related to the hospice diagnosis are provided and coordinated by our team to support quality of life and peace of mind.

Medical Equipment

We provide all medically necessary equipment to support comfort, safety, and mobility at home. This may include items such as a hospital bed, oxygen equipment, wheelchair, walker, bedside commode, and more—based on each patient’s individual needs. Our team coordinates delivery, setup, and ongoing maintenance to ensure everything is in place for optimal care.

Spiritual Care and Counseling

Our Chaplain provides compassionate spiritual care and counseling for both patients and their families, respecting all faiths and beliefs. Support is available for navigating end-of-life concerns, emotional challenges, and personal reflection. The Chaplain is also available 24/7 for crisis intervention, and can assist with planning and conducting funeral or memorial services.

Bereavement and Grief Support

We offer comprehensive bereavement and grief support to the patient’s family, both during the hospice journey and after the loss of a loved one. Our team provides counseling, educational materials, and compassionate guidance to help families cope, heal, and find strength during this difficult time.

Physical, Occupational, and Speech Therapies

When appropriate, physical, occupational, and speech therapy services are available to enhance the patient’s comfort, independence, and overall quality of life. These therapies are tailored to each individual’s needs and goals in coordination with the interdisciplinary care team.

Volunteers

Hospice Volunteers play a vital role in providing companionship, emotional support, and practical assistance to patients and families. Whether offering a listening ear, helping with light tasks, or simply being present, their compassionate presence brings comfort and connection during life’s most tender moments.
